The extension also needs additional Java component (Java JRE SDK) to be installed in order to work. I got it working after downloading the latest version here: https://www.java.com/en/download/manual.jsp Hopefully this will help.
It does not work and I even think it might be a scam. In the menu (click 3 dots then manage extension) I saw the settings of the extension. "Site access'' This extension can read and change your data on sites. You can control which sites the extension can access. My experience with eID ranges from "An absolute pain to get working" to "Impossible to get working".

Hoi, Bij deze een vraag aan de softwareontwikkelaar om de documentatie van de extensie te vervolledigen. Voor je de extensie kan gebruiken, dien je eerst een andere software te instaleren die als tussen-appplicatie fungeert (https://www.e-contract.be/eid-chrome/releases/?C=M;O=D). Daarna kan je de chrome extensie pas gebruiken.
The extension writes a <div id="eid-chrome-extension-is-installed"></div> in every web page you visit which could lead in unexpected UI alterations. I suppose the idea is to detect if the extension is installed, but this is not the correct way to do it. Sorry but I can't trust a company/extension doing this, especially since we gave them access to our EID card.

Stay away. It does not work, but that is only the least of problems. It looks like a very amateuristic piece of programming. I would not trust this thing with my personal data in a million years.

installation failed on macbooks so extensions could start reading. on windows 10 it seems it was installed, but could not find reader as well.
