It does not replace images with placeholders - It hides the original elements with display:none; and then pushes a new element next to the hidden tag (with their own styles..), effectively breaking the webpages and flooding it with 2x as many images. It also has to be enabled on each page load, and a website needs to be fully loaded in order for the plugin to do its job, so you will see the images while the page is loading for a few seconds, and then it breaks.
A bit glarish in itself. I would prefer a browser override to start with.. all pages on or off and go from there, but this sets everything automatically off, puts its logo everywhere, and each site has to be set manually each time with no saving feature.
